Filling out PIP advise...as me talking or DD?

Ok so finally doing my DD's PIP assessment. She is 16.
Do I write this as if she is writing (mum helps with hair washing and supervises me brushing teeth) or do I write it as me (I have to wash hair and supervise teeth brushing as otherwise personal care does not get done).

It's funny when you actually get to do the form I am struggling to word the constantness (word?!) Of looking after a child with asd and adhd....she manages some aspects of "mainstream" life but not without constant supervision and support.

Dh is Ds's appointee but I filled the form in. I actually wrote it as if I was Ds. So said 'I need support with this' etc. Afterwards I did wonder if it should have done it that way or if I should have said 'we support him with'.

We have just found out he is eligible for enhanced daily living and enhanced mobility and they didn't do an assessment with him. They just contacted one of the professionals he works with. So I'm guessing the form was fine.

I think you will be fine doing it that way. Ds is autistic and I was just wrote down everything he needed support with. Some things he's okay with, like using the toilet, so I just left that blank. We were fortunate in that we had a health professional that contributed to the additional information on the form, so the assessor approached them for any extra info and did not make any contact with us. I also sent off any assessments and letters that I thought would be relevant.

It did take very long time to hear anything. We phoned to start the claim in October last year (we had not claimed DLA previously for Ds) and we had the letter last week to say he was eligible. So 8 months from start to finish.

I filled it in from my point of view. If I'd filled it in from my daughter's point of view I'd have been worried that they'd have thought she was capable of filling the form in.

Definitely write it from your perspective, otherwise as Punx and Hally posted, there's the worry it may be used against you and DD will be seen as more capable than she is. If you haven't already apply to be appointee.

Punx brilliant to hear something has been relatively straightforward for you, and DS wasn't subjected to an assessment.

Thanks 10green I'm glad this has been relatively straightforward. Ds was surprised that he was entitled to enhanced daily living and mobility. But I told him they do not give PIP out easily, so he is absolutely entitled to it.

I only wish that sorting out education and health was more straightforward.

It sounds as if people have got pip doing it both ways. I wrote it as 'I do x, y, z for dc'. At the back I documented I'd filled it in on dc behalf as dc was unable to. We then both signed it as I wasn't sure who should. I wanted to get across that dc was unable to fill out such a form.
We got pip awarded high rate care and low mobility (which is what we expected) following a short telephone conversation.

Landslidelife if you are DC's appointee you can complete the forms and speak on DC's behalf. If not DC would need to, but I suggest you ask to become appointee.

I do ds and dd's in third person. Eg. (Name) is unable to wash her hair and needs help from her mother to do this.
They are both due for renewal in the next year so I haven't done it for a while.
