Advice on filling in PIP form

DS is now 16 and we need to complete a PIP form as they want to transfer him over from DLA.
Given everything that has come to light about the use and misuse of information in the assessment process I can't afford to make any mistakes in completing it. Does anyone have any experience of the process, and advice?
I am very nervous of all this.

hi I filled in one of these forms about 2 years ago. I filled it in as honestly ad in could. sometimes when you write down the difficulties your child has it seems worse on paper. just be honest about everything. they will get a statement from the senco at school also.

I've recently filled one in for my DD (a couple of months ago) the advice I would give is to be honest and to give as much detail as you can, even adding the little things that you may not think would help. They will cross reference with health professionals etc so include any up to date reports from them if you have any.

I have yet to do this. I kind of gave up any hope after they took away higher rate PIP for my disabled husband, bed bound, and the assessor lied on his report.
I also don't understand when they have to be in education to 18, why the DLA does't continue to 18. Yet another way for the lovely government to negate their responsibility and save money.
Sorry, bit of a bug bear of mine this one.
There's a really great organisation that has a Facebook page: Fight Back, and they are really successful at helping people fill in their forms and then to appeal if turned down. They are a charitable organisation, ask for nothing, but a donation is always welcomed as I think they are manned all by volunteers.

Remember to include as much supporting evidence from professionals as possible!
