Anyone done a moving on plan/LDA?

ds2 had his Y12 annual review this morning and is going to be leaving his sn school to go to college so the statement will cease

the teacher at the review said that a woman called Anne would draw the plan up. she didn't know who this woman was but assumed she was someone from connexions or the LEA or the local careers office but wasn't sure - just that the woman was called Anne. Apparently the head of 6th form at school sends off the information and this lady puts it all together for me to look at.

Is it like a proposed statement at this stage (I know there is no legal backing to it) but does anyone know what entitlement there is for me to change/reword/add anything that I disagree with

My DS has just finished his GCSE's and is due to go to 6th form college in September. His statement will also cease but way back in November we met with someone from young people's services and discussed what DS wanted to do next, support, needs etc. She was present at his annual review after Christmas where we all talked about his transfer and transition to college. This what all put into a moving on plan ( it has a weird code SD 492A or something similar). I was sent a copy to proof read and make any changes. She made reference to his statement and it was clear that she had listened very carefully to everything that had been said and discussed. Once DS had applied to college, a copy of the plan was sent to them and they said wether they could provide what he needed. Happily, the local 6th form college has an amazing SEN department and they are able to accommodate him.
I was advised to make sure that EVERYTHING he could possibly need was put into the plan because it would be too late to request stuff I hadn't thought of later.

S139A is what the form is called.

Anyone else? My daughter is in year 11 and we are about to begin this process.

Ds's Section 139A plan was started a year ago but under different circumstances as he's home educated. I approached Connexions to start the ball rolling after making enquiries about college options for students with autism/adhd. Ds never did get a statement while at school but one was finalised for him in July this year, so he's now 'in the system'.

My LA is also a pathfinder for the new single plan so now that we have the statement we're working through the process for this. Section 139A will be replaced by the birth to 25 years single plan from Sept 2014 so this will (hopefully) ensure that ds's needs are covered.

He's nearly 15 btw, so would be Y10 I think.

We would like my daughter to carry on at her special school until 19, it is more complicated because the school is outside our LEA so we have to look at all the FE college options. Does the statement cease even if she were to continue at school.?

There will be NO statements from September 2014 as they'll all be replaced by single plans, which are intended to cover support from birth to 25. I'm not sure how it will affect individual LAs or schools so it would be best to check it out directly.

It is such a minefield isn't it.
