I am in similar position, awaiting mine, but that is what I am expecting, so think you have right thing. parent partnership been brill for us, with lots of advice on how to complete, hope that's useful info... good luck

Each area does it slightly differently, as usual. Ours was a 4 page doc, iirc. Can't remember when I was in Kent, its so long ago.

I ignored the form itself, and typed it all up in Word, using the same questions - each one on a new.page. I think our parental contribution was 28 pages long in the end. I am happy to send via email, if you think it would help to have a read of one (not that ours is perfect!)

I think you should also have had a pack outlining the process, the appeals process, etc. Lots of different leaflets, and flow charts showing the date deadlines etc.

Do you have a named case officer?

To be clear; I presume from your OP that you have had agreement from panel to perform a statutory assessment.

If so, they are simply asking for your opinions and wishes. So think carefully about what these are before filling in forms.

My understanding is (having gone through this with multiple children in multiple LEAs) that the precise nature of paperwork is LEA specific. So whilst I haven't had a big pink form to fill, I have had what I believe to be the equivalents.

I would suggest that before you touch the forms you create a draft, and then discuss it either with an experienced friend or acquaintance, or with a body such as IPSEA. Or both, prior to submitting...

...and the honest truth here is (sad to say) if you don't have to exert pressure/haggle vehemently/complain bitterly/go over case officer's head..

..then you are being sold short. Even when all involved are trying genuinely to be helpful, they always need pushing...

Thanks signandsay, the lady from PP did offer her help previously, so i will take her up on that.

Silverfrog, that would be great and lovely of you to offer. You have my email i think? No case officer.

Retired, yes thats correct LA have agreed to assess. I will contact IPSEA and as for my experienced friend, that would be MN!

Absolutrly first thing I would do is get on the phone and find out case officer. Once I had a single person to harass check in with daily, I found things went a little smoother (or maybe I just felt better as I could pointedly irritate someone in the way they were doing to me )

Will send email in a bit - need to stop the dds badgering each other - the joys of half term!

Our form had the questions copied from Chapter 8 of the SEN Toolkit. If your questions aren't the same, I'd suggest looking at the SEN Toolkit questions as well.

I also ignored the form and typed pages and pages worth. The LA has more or less ignored most of it, but I'm glad I went into so much detail as it gives a much broader picture of DS' difficulties than any of the professional reports - and the prospective schools and tribunal will read it, even if the LA don't bother.

Sugar, thanks for the link, those are the questions. Will have a good read of the link and advice tonight.

Yes, form doesnt give you much space does it!

Silver - thank you i have received your email, will have a read later tonight as well. Thank you very much for sharing this with me.

Oh those are our questions too - no surprise that LEA don't show any originality!

No problem, claw. It doesn't say anything that I haven't said on here, I don't think, just lumps it.all together and paints a gloomy worst case.scenario.

Has been quite refreshing reading it, actually, as dd1 is nothing like that at all now - show just how right we were in our school choice!

Silver, great that you have seen the improvement and thank you again.

Must get on and do some housework and get ds out of the house for a few hours, will have a read later.
