~Anyone have a care plan?

What is or can be included in a care plan?

School nurse is phoning me on Monday to discuss a care plan. Ive tried to google examples, but cant find anything.

I've only ever come across care plans for children who use epi-pens but it contains a photograph of the child, a summary of any allergy, symptoms of exposure to the allergens and the action to be taken. They are in the school kitchen, office, staffroom and own child's classroom.

My DD's friend in Reception (who has DS and a tracheotomy) will have one which details the procedures involved with clearing/cleaning the tracheotomy.

Claw it can contain things like incontinece issues, there. Normally based around medical or help with personal care

Thanks SE13 and round, sorry i should have said continence is why the nurse is going to write a care plan, i wondered what kind of help ds could receive for this? He needs help with cleaning himself up, i cant imagine any volunteers for that job.

I wasnt sure about whether it could also include eating and dressing.

Hi Leonie, yes i cant imagine them helping ds with or without a care plan or a statement, im just collecting evidence for the LA so i can get a statement and get ds out of there.

Would like to include as many things on the care plan as it covers to add to my list of things the school dont do, as evidence.

Does your dd have a statement?

Leonie, no worries you off load

Its 6 weeks here, so you shouldnt have to wait much longer. You will probably receive it at the latest time possible, they drag everything out, dont they!

Dont underestimate them, they dont give a shit how distressed a child is. You would think receiving a report stating this they would have to assess, but they hang onto their 'criteria' for dear life and would rather delay until it reaches tribunal stage, in the hope they can wriggle out of it somehow.

Ds talks of wanting to go to bed and never wake up, he has scratched the skin off of his face and finger and ended up in hospital almost having to have a finger amputated. He comes home smothered in shit. But LA are saying they need more evidence.

I assume its different to the Care Plan that comes out of assessment by Soc Svs? That details what has been agreed in terms of respite, prices it all and covers the general background/situation of the child. This sounds like something else altogether, how confusing.

Yep it's based round what care needed in school ds will need one , to assist with toileting cutting up of food ,getting changed etc all these are on ds statement to the care plan will just be easier for staff to see and acess

Yes Davros, when i was googling care plans all i was getting were the SS ones

Thanks Round, no doubt ds's school will file it with the rest of the recommendations they dont follow. But im hoping the school will have a total change of heart or at the very least i can use it for evidence for LA.

I thought it was worth mentioning the Soc Svs ones to alert anyone who hasn't got Soc Svs involved in case there is any confusion with using the same title iyswim.
