3rd attempt at OT report and its still crap

Have been emailed 3rd OT report and its not much better than the 1st or 2nd one.

OT who did the assessment in 2008 is on long term sick.

So report is based on notes from 2008 and a different OT going into school in March 2010.

I have told them numerous times that ds experiences lots of problems during playtime and lunchtime ie unsupervised times. So she assesses in the class room where he receives a high level of support and plenty of adults present.

Despite me telling her that he was born on the 91st centile and managed to stay there (diet consists of carbs, fat and sugar) until he started school. He doesnt eat at lunch time and his weight has dropped from 91st centile in April 2009 to 50th centile in September 2009 and a referral to a feeding clinic has been made. she comments that he was seen to eat ALL of his carrot (a carrot stick, not a whole carrot) in the class during snack time. To me there is a big difference between eating in a class room of 30 kids, to eating in a lunch hall, but no mention of anything but a bloody carrot!

I have listed 3 times now, what is wrong with the report etc, etc and its not getting any better.

Do i just tell them not to bother with a report and try to get a proper assessment done elsewhere?

or accept report and file under crap?

Hi.I think there is a big gap between 2008 and March 2010,and a lot can change/happen to a dc in this time.
From what you have written,there seems very little assessmnt but rather observation. What was your ds assessed for? Did the original OT carry out any standard tests on your ds, or just observe him, cause there is a difference.
If this is the 3rd write of the report, and you think it innacurate, you could/should complain to the Head of your Primary Care Trust, and to the NHS complaints ombudsman in your area and cc in the Head of the OT dept, so they know you mean business.
I would request a fresh assessment of your ds in your complaint, and depending on the outcome of that, what future OT input will there be?
Point out in your complaint that your ds has been referred to eating clinic for weight loss, and say you had hoped the OT could have offered guidance on sensory issues and where the least stressful place for your ds to eat at school could be. Good Luck!

Thanks Bigpants, i had to make an official complaint in August 2009 to Primary Care Trust when 1st OT stopped going into school, didnt prepare sensory diet or write a report as she said she would and wouldnt return my phone calls. She has been on long term sick ever since! hence the observation in school in March.

He was referred to OT by SALT in 2007 for sensory difficulties (particularly around his eating)

No standard testing, just clinical observation about 4 x 1 hour sessions in 2008. Notes seem very vague to be honest.

I requested that if this information from clinical assessments was not available to them, then they should reassess.

I have moved boroughs and they are trying to discharge me, so they dont want to reassess.
