Transition Plans - what is your child's plan like?

We had DS1's Transition Annual review last week and we have just received his Transition Assessment and...er...'plan'.

All it is, is a CAF form, with the usual reports you expect from professionals (SALT, Sensory Needs Teacher, Class Teacher, Physio), cut and pasted onto it.

There is no plan

Our 'Lead Professional' is cited as DS's school.

There is no mention of DS's ASD. The psychiatrist from CAMHS who diagnosed ASD and who sees DS regularly has not been named and has not submitted a report

There is no input from my son or us, his parents. All it says is:

'DS will move to [college] at yr 12. Mum was enquiring about post 19 provision which [Transition worker] will support the family with'.

I feel quite downheartened that we seem to be having difficulties so early on and this 'plan' seems to be so desperately lacking in any substance.

Does anyone know what I should expect from a Transition Assessment and Plan before I go back to the school?

ps I hate being called 'mum' by professionals, especially in a formal report!! GRRRRR

grrrr for you, lol. not done one yet, been mentioned but i though they would be done closer to the time dc changes schools, ididnt know they were done nowish.

pokhara, the transition process begins in Year 9 - aged 13-14, horrifyingly enough!

as you know I have been told to wait untill the yr 11 review, and then I will start thr battle to keep dd at her present school.
no mention has been made of 19 plus.
apart from them asking what I hoped for.

Well, I hope your plan has a bit more detail than mine!! It's hopeless. We also never discussed 16-18 provision in the meeting, it is just assumed that DS will go to you-know-where. Think I'll ask for a transfer!

they won't talk about it untill yr 11.
tbh you know where is good, ds raves about it and would like to work there(although he still says dd will go there over his dead body!)
(is your ds booked in at youth scheme over easter?)

He is indeed. Is your DS? We need to make the most of it as the AH money is going to run out by this time next year!

I can't understand why they have a Transition review in year 9 if they don't talk about options until they are in year 11! What is that about?

I'm going to put DS down for Eton

Next week I understand the Dyscovery centre are coming in to trial new transition pack they are working on for ASD kids (they do courses etc at their abse in cardiff), it apaprently is 6 months from launch but I will make sure I sahre anything gleaned of note

Have apparently got some notes on ASD post 16 education on their way to pop on TTR, I will keep an eye out for when it arrives- are you on TTR DD?

Did I write DS? whoops. Of course, I know she's a DD not a DS - just a long, long day.

Not on TTR 'fraid. I've got some transition stuff from Autism Education Trust, but I think I need to show someone the form and ask if it's acceptable. Thanks all

I've got a sample post 16 IEP and assessment on tehre but the other stuff won't be in until next week; I am presuming 2shoes has your email so maybe she could send the files over (if she doesn't mind?)
