I have to fill in a form for school...

I found a large brown envelope in my son's reading bag today, it's a form from the National Strategies Primary and Secondary Inclusion evelopment Programme. There was a note with it from his teacher (also the senco) asking me to fill in any parts of the form that apply to the home. Now looking at this form I'm guessing it's for the teacher to fill in, and now I'm a bit confused.

There are questions on it such as: What is your biggest problem teaching this child?; What are the pupil's most appealing features?; along with all sorts of stuff about ability to dress/undress and academic stuff etc.

Has anyone else here had to fill in one of these? And does anyone have any clue as to what it's for? Ds is 6 and has verbal dx of aspergers.

Also, do you think I should fill in my answers in pencil so she can erase them but use them to form her own answers?

i always write it on a sep sheet, it just sounds as though it's for an external referral (ed psych, outreach team maybe?)

either way, it is good that she's asking for your input. it sounds as though she is keen to get more support in place and keep the verbal dx moving forward (or gather more evidence as appropriate)

i'm guessing she doesn't want your opinion on the 'teaching' stuff, but probably on the self-help skills (dressing etc) but i woulkd be using the opportunity to make sure she knows as much about ds as poss

Thanks madwoman. I shall write my answers on a sep piece of paper. Hopefully it IS for the ed psych, as the paed has written to her to refer him to ed psych.
