Seeing SenCo in new yr, need help.

My DD (11yo) has a number of dxs, and has been on SA+ since she was 3.5. She got next to no help at primary (personality clash with that SenCo). She went to MS secondary in September, and I found out last week that although I put the basics on her entry form, hardly any of her dxs were in her SEN records from primary. I have an appointment with (a much nicer) SenCo in Jan, but I don't know where to start! DD was dxd hypermobility and GDD before 1yo, hypotonia at 18 mo, partial deafness at 2yo, ASD at 4 yo, heart probs at 9yo and epilepsy at 10yo. She is having probs at school, but just don't know where to start. I tried to get her statemented at primary (in Y5 & again in Y6, but LEA weren't interested as primary SenCo refused to back me 'as it was too much paperwork [hmmm] ). By the sounds of it, the Secondary SenCo will be much more receptive to this, does anyone have any advice for me?

I should add, that she hasn't made much progress in 8 yrs on SA+, she is in Y7, but is currently working on lvl 2/3 in all subjects ( and that's only since she went to secondary, in July she was lvl 1 in English, and still on p scales for maths, like I said, primary did bugger all nothoing to help). She's also having probs with the social aspects of MS. I personally have ALWAYS felt that special school would be better for her, there are 2 very good ones locally, that both offer the chance of gcse's in Y10/11, so if she was able to do them, she could. Just not the 11-12 that her MS school would try to get her to do...
