School note of concern...what exactly does that mean for dd

hello again all you wondeful experienced mums LOL
I have been continually pestering speaking to DD3's teacher over lastfew weeks as I am increasingly concerned about her lack of progress(for those that dont remember my last thread DD3 is 5 and we have a lot of concerns about her....long story short am thinking asd...maybe milder end of spectrum. we are waiting for an assessment for her, but also she is significantly behind at school)
Anyway she has finally stopped tellingmethat she may justcatch up, or that it is because she is young for her year (end JUL bday)...and is going to complete a note of concern for her....
So what I wondering iswhat exactly does that mean? They are doing someextra phonics with her...but nothing else as far as I know...
What would be the nextstage from a note of concern? Any info greatly appreciated as I really dont understand how the whole school special needs stuff works.

Note of concern sounds like our Nature of Concern, it's the first flag so that her progress in the named area can be informally monitored.
Next step is an IEP at School Action level.
If extra agencies are called on for advice and support it goes to IEP School Action + After that it's a Statement.
Up until statement level nothing is legally binding on the school, and support can be added or withdrawn as they see fit.
