My DD has had her app with the kidney dox today - and here's me trying to be prepared for a more general appointment about getting her an assessment (app was scheduled for her normal consultant - they changed it at the last min)

So, kidney doc as arranged for another app for 4 weeks, where i can discuss DD's difficulties. He has instructed me to obtain a report from her school detailing how she is at school. So, i am now appealing to other m'netters to help me try and figure out how to ask school!

DD was diagnosed with sensory processing disorder in 2004, but apart from a little work with the EP (circle of friends group) she has had no further input from anywhere. So, she has fallen further and further behind her peers - she is now 12. School are aware that she has difficulties in mixing, following instructions, clumsiness etc, but no statement.

Her OT did mention that she had a few aspergers/dyspraxic traits, so it is this that i would like her assessing for really, but not sure how to set about it. When reading other posts on here, and from working with other children on the AS, i can see that she is very similar.

BUT, i am not sure how to word letter to school - in that i dont really know what is important to the consultant or not? I was thinking along the lines of a list of specific questions - perhaps to her class teacher of head of year, as i know she has only seen the SENCO twice. But should it go to the SENCO???

Sorry for the long post - turned out a lot longer than i thought lol! Anyway, what i am really asking for, is for help in choosing which questions to ask school?

Hi, does your daughter have an i.e.p? this will be useful to show doc. My ds' psych contacted the school directly and asked specific questions about socialising with other children, any challenging behaviours, verbal ability/ communication skills, etc., also acedemic level/learning ability.

no, no iep. she was on school action plus at primary, but since she started Y7, nothing.

And that does help, thanks very much. Am just sitting staring blankly at computer, as really want to try and get something in for tomorrow, to give them plenty time to write something!
