Request for formal assessment
I am writing as the parent of the above child to request an assessment of his special educational needs under the 1996 Education Act.
Jxxxxxxx will attend Lxxxxxxxxxxx school Sept 2009.
I believe that Jxxxxxxxxxxx special educational needs are as follows:
He is Non-verbal, has global development delay, Autistic features, learning difficulties, and has very little understanding of spoken word. His social interaction is extremely young and he cannot interact with his peer group.
My reasons for believing that the school cannot on their own make the provision required to meet my child?s needs are:
Jxxxxxxxxx does not understand spoken instructions with result that he cannot carry out requests set by the teacher, or indeed understand them, and needs someone to stay with him to show him, and teach him. Jonas needs constant encouragement to stay on task, as he has attention deficit. He also needs a teacher trained in Makaton sign language as that is his only form of communication. He also suffers from Pica, an eating disorder where he will eat non foodstuffs, and as he is wheat/gluten/egg/soya/dairy intolerant, he needs someone to constantly watch that he isn?t not only eating foods which are not safe for him to eat, but also things that are dangerous or poisonous should he swallow them. He has no sense of danger for himself or those around him and needs constant supervision.
I understand that you are required by law to reply to this request within six weeks and that if you refuse I will be able to appeal to the Special Educational Needs Tribunal.
