letter for "formal assessment" statementing starter - can someone have a quick look please?

written the letter (pinched from IPSEA - thanks attila!) but not sure if its the sort of thing that im supposed to write:

Dear Sir or Madam,

Jxxxxxxxxxxx D.O.B xx/xx/xx
Request for formal assessment
I am writing as the parent of the above child to request an assessment of his special educational needs under the 1996 Education Act.

Jxxxxxxx will attend Lxxxxxxxxxxx school Sept 2009.

I believe that Jxxxxxxxxxxx special educational needs are as follows:
He is Non-verbal, has global development delay, Autistic features, learning difficulties, and has very little understanding of spoken word. His social interaction is extremely young and he cannot interact with his peer group.

My reasons for believing that the school cannot on their own make the provision required to meet my child?s needs are:
Jxxxxxxxxx does not understand spoken instructions with result that he cannot carry out requests set by the teacher, or indeed understand them, and needs someone to stay with him to show him, and teach him. Jonas needs constant encouragement to stay on task, as he has attention deficit. He also needs a teacher trained in Makaton sign language as that is his only form of communication. He also suffers from Pica, an eating disorder where he will eat non foodstuffs, and as he is wheat/gluten/egg/soya/dairy intolerant, he needs someone to constantly watch that he isn?t not only eating foods which are not safe for him to eat, but also things that are dangerous or poisonous should he swallow them. He has no sense of danger for himself or those around him and needs constant supervision.

I understand that you are required by law to reply to this request within six weeks and that if you refuse I will be able to appeal to the Special Educational Needs Tribunal.

sorry i have no idea as never had to do one, yet! i would say though maybe on the school dates put something more like due to start in his first primary year on... so its clear this is his first start in school as some people just cant add up from dates!! otherwise sounds good to me but as i say i have no idea what is needed. sorry

ooops! oh well, its been hectic this morning!

gonna take it across to school and mention that i am definately going to go forward with it. I have been umming and errring for 2mths!

After filling in the latest DLA form, its become fairly obvious that hes going to need one, just a bit of a battle ahead now...

the first letter i sent was standard like that one

tjhey then sent me out a pack to fill in im sure it was ds who was supposed to fill it in but due to age couldnt so i wrote it out as it was like his personal passport how things are seen through his eyes his struggles his likes his dislikes what bothers him etc

they then write off to all professionals involved with him for there reports and then you just wait im still waiting for our proposed statement think i have few more weeks of waiting left all reports had to be returned to them by 30th march so not sure what the waiting time is now

my pack that got sent had to be returned within 6 weeks im sure i had 2 things that i had to fill and send back

good luck with it , and yep teh next pack through will be the one for lots of detail, then there as you almost the same thing for the next stage

Send it recorded delivery to the SEN Section within your Local Authority - not the school.
