Child let himelf out of school

DS is 10, autistic, adhd and complex needs. EHCP and full-time 1-1.

Highly struggling at the moment and a known flight risk. School walk to Church once a term. He doesn't walk due to risk assessment but goes on minibus. This bit is fine.

For whatever reason today his 1-1 didn't go with him and got used to supervise the walkers. This was OK on the bus but when he got back to school he was incredibly dysregulated, left to behave dangerously in the school car park , then unsupervised in school as there was a good 10-15 minutes before the walkers got back. Eventually, DS let himself out of school on to the carpark, then got angry as their brother wasn't back so we couldn't leave so bolted back indoors and was dysregulated within school and not an adult in sight. There was nothing stopping me follow him in either which would have meant a random adult walking about. Eventually took themselves to the music room to calm themselves when an adult spotted them but this took place over a ten minute period.

Is this not a major safeguarding issue?

Yes. Write down what happened and ask to see the HT.

This should not have happened. DS should not have been left alone. He should not have been able to leave the building. If 1:1 is detailed, specified and quantified in F, the 1:1 should not have been used for other duties. Request a meeting with the HT and SENCO. Make sure you follow everything up in writing. If the 1:1 is detailed, specified and quantified in F, remind them the EHCP is a legal document.
