ADHD assessment in 12 year old....

We've thought for a while that our daughter could possibly have ADHD.

Things we have noticed:

Constant talking (has been an issue since very little) becomes unbearable to be around sometimes.

Along with the constant fidgeting, rubbing feet together whilst watching tv, constantly bounding around, humming, singing/random noises.

Trouble remembering more than one instruction at a time, forgets simple tasks, is careless with her things, looses everything, has to be constantly nagged to do basic things like getting dressed, brushing teeth, several trips back up stairs for forgetting to put socks on or bring a specific item down.

Minimal effort at school work, careless mistakes, struggles to maintain focus with homework, doesn't recall learning things at school which I think is down to her lack of ability to concentrate.

I'm sure there are other things too. Teachers from school always say she is enthusiastic to learn but doesn't apply it to her written work etc

What would be the best way to approach this without making it something for her to worry about?
Do I speak to the GP first? Can this be done without her present to discuss concerns before diving into anything?

If you are concerned you can speak to the GP &/or school SENCO. You can initially do this without DD present.
