DLA- help.

Hello.
My daughters point of contact with portage has recommended that we apply for dla. She has offered to help us with the forms so the filling out shouldn't be too difficult.
However I have a crux, in being the silly twit I am I have misplaced the majority of her medical letters that I would need to provide with the application. Does anybody know if I can request for these to be resent out? And if so who do I contact for that? I'm a silly billy I know 🤦🏻‍♀️

You can access her medical records via your GP surgery and ask for copies of letters from the respective consultants to your GP.

You'll need all the medical evidence she already has plus any new evidence as she gets older for different things, so keep everything together in a safe place from now on.

It's much better to familiarise yourself with the forms rather than rely on a third party filling them in, they won't be able to answer the questions without a lot of input from you anyway. Also it depends whether you want that person to know all your daughter's confidential medical details.

Before you send the completed forms off, take a copy and keep it safe with all your daughter's other documents.

Send the forms at the Post Office and use their 24 hour Tracked mail.

Yes I've already downloaded to forms so I can go through the questions and start assessing my dd day to day.
The lady already has all dds medical records, as she works with her paediatrician/speech and language therapist etc. So all relevant information regarding dd and why she'd need DLA goes to her with everything else.

Fabulous, it sounds as though you're on the ball!

Never be discouraged if they refuse at first, appeal and appeal. My DD is adult now, so my DLA days are in the past, it's all PIP now for us, but I have heard that DLA is becoming more difficult to get. Don't be discouraged, don't give up Flowers

@BlankTimes thank you again. Yes she mentioned that we could have a fight on our hands, as they try to discredit a lot these days! Thankfully I've had lots of practise fighting for support since she was born, so I've gotten very good at getting her what I believe she needs!
