Poor account conduct

A brief background. I'm 39 and OH is 38. I am the main earner and he works self employed with earnings around £11500 per annum. We have a 10% deposit and have started thinking about mortgages. I called the bank today (as 90% LTV's are scarce right now). The checked the figures and said we qualify and ran it through for an offer in principle. Sadly it was declined stating 'poor account conduct'. The bank couldn't explain it stating there's nothing on my account which would prompt that. I have an excellent credit score whenever I run it through and have a credit card (with 0 balance) from high street lenders.

The only thing we can conclude is the issue is with OH. However we have run him through Experian and there are no issues or alerts. We have run Equifax and there are no alerts however on the report there is one old PayPal account that defaulted in 2018. He does not use his overdraft (£750) but does have one capital one credit card with a balance of £750 (it's actually about £150 as we cleared it yesterday but for the sake of a credit check that won't show yet). He was bankrupt 12 years ago and has spent the last few years slowly building up a credit score (hence the capital one card). He has a van on finance for work which is nearly cleared.

I guess my questions are

  1. what else could cause the issues of poor conduct?
  2. what can we do that we haven't already to improve it?

Any thoughts or advice are welcome
