Presumably you reside in England/Wales as the situation is different for Scotland.

You will need to write to the Chief Education Officer of your Local Education Authority (LEA) - you can get his/her name from your local council offices. You will need to give the LEA six weeks to apply (mark that date on your calendar).

You as the parents are far better off applying for such a document as you can appeal the decision in the event the LEA say no to assessment. School cannot appeal if the LEA say no. BTW you must appeal if the LEA say no. Do not give up.

IPSEA are very good at the minefield that is statementing and there are model letters on their website that you can use. It also tells you more about the whole process.

Also, speak to the charity for whatever Sn your child has if you can- they often offer excellent ideas on specific things that can be requested / suggested, for example the NAS has an education helpline that is good. Also, if there is a parent support group (we have SNAP here, it varies) in your area enlist their help as well.

As Attilla says, appeal if you get a refusal- indeed, almost expect a refusal or inappropriate statementa ts ome point; its almost de rigeur to senfd them out- don't be phased by that at all.

And do be totally honest- statement application, like SLA forms, are not the time to put 'Ah well we love hima nd he's OK really I just think he'd be a little happier if...#- state the truth, worst case scenarios, and state them with strength and conviction!
