Referral to paeds but what could it be?

My dad age 8 has been referred for a paediatric appointment due to many issues to see if there is something that is going on to cause them. But I have no idea what they might be looking for.
Some of the things identified are: he is very behind in all things academically, just finishing yr 3 but mostly working at reception level.
He has a terrible memory, he will ask me the same question several times over the span of a few minutes and genuinely can't remember the answer I have given. School have also noticed this and say that he will have a one to one session with their specialist teacher one day but by the next has forgotten everything he learned the previous day.
He is quite ridged in the foods he will eat, he has had a cheese sandwich nearly everyday since he was 2.
He is apparently well liked by the other children in his class but when I have observed him he doesn't interact with them very much.
He can't draw or paint anything recognisable.
He seemed to hit his milestones as a baby and I have only been concerned that something is different in recent years.
He regularly says he doesn't want to go to school, although he never refuses to go he says he just wants to stay home but can't verbalise why.
He seems to have a lot of trouble verbalising many things thinking about it, I am often trying to work out what he is trying to get at.
I am sure there are more things but those are the main ones.
I have three older children and two of them are dyslexic.
Can anyone help advise me in what the paediatrician might be looking for?
This is all new to me and I just want my lovely little boy to get whatever help he needs.

you took him to the GP for a referral? What were your reasons? and who are you seeing?

Sorry no the school have asked his go for a referral as they want it explored.

All it says on the letter is that it's an appointment to see a paediatrician, no further details

I would either self refer DC to the speech and language therapy service and occupational therapy service for assessment; or ask the paediatrician to refer DC.

I would ask the school to get the LA educational psychologist to assess DC, and as well assessments on reading, spelling, numeracy, etc, in particular look at memory. Afaik, speech therapists can assess auditory memory; but educational psychologists can do more comprehensive assessments such as on visual memory, long term memory, etc too.
