ASD/ADD template/checklist to take to GP?

We have finally made the decision to go to the GP with our concerns about our DS and I feel like I need to document some things first as I wouldn't be able to articulate everything very well under the pressure of an appointment. Can anyone recommend a template or something that they did to take with them?

I wrote my GP a letter which I handed in a week or so before my appointment. I listed our concerns about DS, why we thought he had ASD, why we wanted him assessed etc. It was about 5 sides long in the end, with examples of behaviour, all bullet-pointed etc. The GP had read it before the appointment, and he only had one or two questions and then said he would refer for assessment.

List ways in which he is significantly different to his peers.

Does he have siblings? Lots of professionals try and say poor parenting is a factor, but if you parent siblings the same way and his behaviour is significantly different to theirs, that should also help your case.

Thank you. Sending a letter first is a great idea, I will do that. He does have a brother but he is younger so it is hard to compare.

Well, we still aren't sure it's not our parenting! But I have read a lot and brought around skeptical DH, one thing in particular I read today which was written by an autistic (PDA) adult who described our issue with DS and the toilet to a tee!
