Placement not viable

If professionals had decided that a school placement was no longer viable and had ?failed? would you have expected to be advised of this?
Back story is that ds with a sm and dx was struggling. Apparently it was considered whether to consult experts in his condition to give advice but this was ruled out due to the placement failing. He was then permanently excluded by the school and left without a placement for 3 months.

I would expect to be informed of problems before it even got to that point.

But I'm funny like that.

Guidance on exclusions says that where a child with a statement is heading towards exclusion the school should be looking at emergency reviews and generally whether the support they're giving is enough, in order to try to avoid exclusion. Certainly if a placement is failing and the expert can see that, I would have thought his/her professional duty would be to refer for more advice as to what would make that placement work or, if it is never going to work, what sort of alternative placement should be organised.
