Does anyone know anything about the Transitional Assessment Guidance Form and its contents?

Ds yr 11 has just had this done for his transition to 6th form (different school).
While what is on it is fine (it should be as it repeats information from the AR from his very good current SENCO and from my parental contribution), much of it is blank as he has been assessed as not having complex needs.
The SENCO assures me that in this context 'complex needs' means multiple severe disabilities and is not relevant to Ds. In other contexts however (CAMHS and SS) he is classed as complex and the form seems to me to be covering more than just his education i.e. transition to adult services.

I am also rather annoyed that the transitions officer from the careers service discussed it with Ds and got him to sign it without contacting either me or the SENCO first. In fact I wouldn't have seen it at all if Ds hadn't told me about it and given me a copy of his copy. Ds is 16 and academically able but he is not able to deal with all that transition involves on his own and frankly I think few 16 year olds would be.

So am I right to think I need to pursue this further?

God, it gets tricky as they get older. My NT 15yo has all sorts of stuff to make decisions on himself, and that worries me. Surely if he's under 18 and has SN they have to still go through you? Sorry no help, but I would pursue it.
