I have got ds's (1st) DLA form in front of me and have just realised why it is taking me so long to fill it in !!

I photocopied the form in order to fill it in .in rough ...but I now have sooo many notes ,extras, add to's and so on (once i read it back to myself and my friend read it ) I still don't know how to fill it in neatly and comprehensivly(sp).

I have so much to write to explain it properly but the form only seems to give space for the bare minimum(is it a ploy !!) .

Isuppose what I need yo know is ...do you write in the margins which would be easier or add an extended "essay!" at the end ??

Going to school now but TIA if anyone can help .

You don't have to write your answers in the spaces - I word processed my dd's recent claim on the advice of Lougle I think it was. This means not only that you can write as much as you need to but also that you have a saved document to use and refer to next time.

Yes, there is no need at all to stick to the spaces.

You have several options:

  1. Hand-write it, then say 'please see separate sheet (page x) for further information.' Ensure that for each question you give extra info for, you label it. So, for example: Question 2, getting dressed - further information. Make sure you photocopy it after.
  1. Word process it, then either write 'see separate sheets' and type it all in a document which you append to the form, or stick your answers in. Make sure you photocopy it after.

What I do (which is a bit anal) is, I type it out, then stick each answer in to the relevant box, folding the paper if it extends beyond the box. That way, I know they a) have all the information and b) can't get 'confused' about which question I was answering. The DWP love me Wink If there is an answer which extends to several pages (for instance, mobility, where DD is not a clear cut case and can be extremely variable) then I write 'see separate page'.

.also photocopy if writing by hand so should you need to go to appeal you know what was in the origional. These forms are a nightmare, but easier if you know what you wrote the first time around!

It doesn't need to be an essay, just use bullet points as I think it is easier for them to pick out the facts

mysleigh we have numerous old forms (first claimed when ds was a baby and hes now 14!). THey are a pain to copy though!

Thanks !!! It will be by hand and I thinnk I will end up with extra pages for nearly every question .!
