Applying for Statutory Assessment

I am going to be applying for SA for my DD. I have gathered all info together from reports/assessments - obviously this list is without my report but i'm feeling rather gloomy now :(

DD is 11 and due to start secondary school next week (eeeeek! ). She is under the following experts (I have also put the reasons why) and i'm wondering if her needs will be classed as 'complex' and would you say i'm justified in thinking she really needs a statement. Here goes:

Camhs - high levels of anxiety, leading to her getting very controlling of me (more than DH) and it's affecting the family as a whole as we can't do things together because of these (very real to her) worries. Lots of time off school towards the end of Y6 as her anxiety got too much for her - Awaiting assessment in October.

Educational Psychologist - This was after them hearing my concerns regarding DD and her full background, since birth. Also worked with DD in school and also noticed 'traits'

Children with Disabilities team - This is the pathway for Autism diagnosis - awaiting assessment

Community Paediatrician - After they met with DD they agreed that she was probably on spectrum (and has all usual 'traits') and would refer to appropriate people

SALT (private & nhs) - Both agree dd's S&L age is 6y5m and nhs lady said she couldn't understand how dd had got through school with no support as she clearly has difficulties

Receptive score = 70 (which is more than 2 standard deviations away from the mean - significant difficulty)
Expressive score = 78 (which is between 1 and 1.5 standard deviations away from the mean - Mild difficulty)
Total language score = 73 (which is between 1.5-2 standard deviations away from the mean - moderate difficulty)

Dyslexia Action - Mild Dyslexia and Chartered Education Psychologist did full cognitive assessment which showed the following:

IQ = 101 and based on the tests taken it shows her weaknesses as:

recall of digits forward = 5 %ile - Age 6years

spelling = 18 %ile - Age 9.3years

word reading = 21 %ile - Age 9.3years

Free writing = 9 %ile - 10 words per minute - Below Average

PhAB Test (spoonerisms test) = score is 86 - Low Average

TOWRE (word reading) = sight word efficiency = 6 %ile - score is 77

Phonemic Decoding efficiency = 16 %ile - score is 85

Total word reading efficiency = 6 %ile - score is 77

ENT - History of glue ear, which causes mild conductive hearing loss in both ears. This does not effect her at home or school (as yet) as she's learned to lip read and found coping mechanisms. Do I even include this if there are no issues as yet?

Sorry it's so long but my DD has had NO support throughout and I already feel like a failure :(

The LA will ask the school for a report and ehat support school has given her to dae, so her current scorings should be provided to the LA by school.

Hope you get their agreement to assess - it seems that she does indeed need additional help.

Think ENT is very relevant to speech and language, reading, and social-communication issues. A mild conductive loss on top of the other difficulties might be exacerbating them. Also worth thinking about auditory processing maybe?

Yes, I have thought about auditory processing too. However, i'm not sure if I need to go back to gp for that or mention to audiology Confused or even if I should self-refer somewhere else!!
