I think it's a standard form, they certainly use the same questions each time in my LA. My DS2 used to answer them in one word answers, What do you like about school? Playtime. What don't you like about school? Writing. What are you good at? Maths. What would make school better? More playtime!
Are these the questions you have? With the DC I support we have a bit of a chat about the questions and then he writes his answers, but, unfortunately, I think he tends to say what he thinks I want to hear. He's very HF, though.
It's such a small part of the review that I think anything you can do to make it easier for your DS will be fine. Maybe give him a multi choice, which restricts his answer but at least gives him the words. Hopefully you'll have some idea what his answers might be, so can include them in your options. Things like, what do you like? Friendships, playtime, ICT, subject strengths. What don't you like? Subject weaknesses, (though they can still enjoy those subjects, of course) sensory problems, too noisy, too busy, long assemblies, long carpet time, friendship problems. What are you good at? Subject strengths, being a kind friend, being helpful. What would make school better? More help with writing, drawing, thinking up ideas, longer playtime, more time to run around, more friends. He could choose more than one answer, or rate the answers with smiley faces, neutral faces or sad faces.
