DLA, quick need help!

i need to put a contact down on dd2 forms. pre-school say they wont do it, and HV has left, my other sister has offered. she is a family support worker for social services and also runs a SN group at the weekend, so is 'qualified' but doesnt see my dd i na professional way, but does 'know' dd2 problems. will dla have a problem with her being my contact as she is family?

why won't pre school do it surely they are the people best place. My dd's teacher did hers

cos they are being a*seholes. they say because dd2 doest have a dx yet they wont do it. they refused to help mieow with her forms for her dd2, even tho she has CP and its pretty darn onvious. pre-school want help in palce for dd2, and want her assessed but wont sign the form.

sorry to hear that. It must be really annoying not having a dx. Sorry I couldn't help. My dd went to a special school from 3 so not had this.

its possible after assessment dd2 will get a SN place at the local nursery in sept. i hope so, as her social skills are pretty appauling atm. as is her eating.

Visit your GP with dd2 and ask her/him to do it?

make a doctors appt a take a writtensheet of all difficulties ect give it too the doctor and ask them to put it on file, then when dla write to the dr, the doc will open the file and be reminded about the difficulties and will hopefull do there report based on what you have written.

Who (proffesional) first picked up her difficulties, could they be the contact, or was it the teachers, if so I think what onlyjoking has suggested is your best option.

Which contact do you mean? There are two names on the form: one at the front(ish) and one at the back. The one at the front should be a doctor or similar and is the person they contact to get a description of the problems. GP or paediatrician or whoever can confirm, preferably someone medical. The one at the back is just someone who knows your child well enough to be a "witness" to the form; they don't need to read it but they need to summarise their conditions and imo it's pretty hard to get it right if you've not read the form. I called the DLA about this bit: I wasn't sure whether to put DD1's Portage worker or her Nanny. They said it didn't much matter as they were unlikely to call them - reading between the lines it's a check in case they suspect fraud.

I actually found the DLA helpline strangely - er - helpful. The number is on the form somewhere.

NB Misdee - don't know where you are based but our local DLA office (Sutton) has closed and forms are redirected to the main office which is slowing things down even further. I don't know if they've closed any others - might be worth giving them a ring before you send it in as it saves a couple of days in the post.
