Hi, i wonder if anyone can help me.
My child has ADHD, Asthma and Epilepsy.
I've had my childs renewal form for a while now and it needs 2 be in by next week.
I'm really struggling at the moment with filling it in.
In the past i've always used a copy of a previous form but althou a lot of what's on the previous form still applies i'm struggling as there is other issues that goes with age.
Can anyone help me please or guide me in the right direction

I copy and paste what hasn't changed and then add in anything else. If his/her needs are the same then age isn't really relevant, unless you are meaning that now he/she is 18 they SHOULD be able to (eg) go to the pub/ drive etc but can't because of their disability? In which case examples of why your child can't do XYZ without support is how I'd write it.

(sorry I'm not sure how different the adult form is as my DS is 14) AFAIK adult DLA is still about the care needed regardless of age or diagnosis:)

Do they automatically reassess at 18? Sorry can't help as DS only 16, but hope someone else can help.

Hi

If you child cannot manage their own affairs, such as dealing with the DWP about the DLA claim form, then you can ask the DWP to be made your child's Appointee.

That means the DWP will recognise you as your child's nominated representative which will make dealing with the DWP a lot easier than trying to get your child to do it if they are not able to do so.

Under 16 the DLA form is all about what your child cannot do in comparision to a similar child of the same age who does not have SENs.

At 16 and above it's not about comparision - it's about what your child cannot do. One important question on the form is the Cooking Test i.e. could your child prepare a 3 course meal from scratch without help.

If not they fail the cooking test and automatically qualify for DLA.

So if your child couldn't do this it's worth taking the time on the form to spell out excatly why the couldn't.
