Parental request for statuory assessment

How do I do this? I've been pushed 1 step too far this morning (although actually its a small thing, think straws and camels backs). I've been on to the council website and there is nothing to tell me how a parent can request a SA. Theres a document that says about schools requesting it with a pile of evidence but nothing about parents requesting it. Called Parent Partnership and got answer phone. Want to do it now whilst I'm in the mood and not being rational about this morning so that it gets done. I've been putting this off for a long time.

check out the ipsea site - they even have model letters to help you word the request correctly.

Yes, you just write a letter to the SEN officer at your LEA requesting a SA for your child.

You don't actually need anything else in the letter at all.

So do I send my mounting pile of evidence now (like proffessional reports, school communications about behaviour etc)?

Thanks for the link. Letter now written. The IPSEA page says I should tell the school. Do I tell the school before I send the letter. I was actually thinking about just telling them I'm done it and its too late to complain because its in the post. Or is that going about it the wrong way?

I told them 'I was advised by parent partnership to', so it kind of made it sound softer.......

just send it, I can't see that school are going to be shocked or surprised you've done this!
