Just completed one for dd1 with SENCO.
Pen profile is one side of A4, with list of dx'es, and areas of differiculties and how to manage them.
The idea is that all staff have a copy and then they all know what to look out for, how to manage child and hopefully have better understanding of child.

DD1 knows this is happening and says shes ok with it, mainly because she hates being told off for not wearing pilmsoles (she has shoe inserts and pilmsoles arent supportive enough) but I feel quite nervous about every staff memember knowing this information about DD. Im hoping they will be understanding but I can see how gossip might get about.
We dont hide the fact DD1 is autistic but not many people know unless they know us personally

I did a communication passport for ds for school. It was about 8 pages of A5. Each one said at least once in CAPs 'I am not a visual learner!' Grin

It was largely ignored.

lisa - surely you really want every member of staff at DDs school to know she is autistic.

I do want them to know to improve their understanding of her behaviour, but I know schools, and can see how it might be a cause for gossip. Also a few parents work there, will tell their kids and I don't want dd1 to be excluded for being the weird kid Sad

I wrote a communication passport for the DC I support and it's really helpful for him. TBH, every member of staff will probably already know about your DD, but now they may be able to support her more effectively. School staff (are supposed to) treat any personal information confidentially.

Trust me, having worked for LA for many years, nothing is 100% confidential Sad

Ds's was ignored too. I wrote the gist of it and the Autism Outreach teacher fine-tuned it to an easy-read format, plus a condensed one page simplified version so everyone who came into contact with him would have a copy. I don't think I met a single member of staff who had even seen it much less refer to it Sad.
