Letter asking for a SA, does this sound ok?

Using the template from ipsea I am requesting a SA from my LA.
does this paragraph sound ok, ds has asd and adhd.

''My reasons for believing that the school cannot on their own make the provision required to meet my child?s needs are that dispite ds having an assessed IQ of 121and being on school action plus the school have told me he has made no academic progress this year. The lack of adequate progress as outlined in the SEN COP leads me to believe school are failing in their duty to give ds an adequate education that takes account of his special educational needs.''

Is this ok or do I need to put in more details, and do I need to quote chapter and verse of SENCOP and should I write sencop out in full or use the letters.

As stated in the SENCOP, ds should make adequate progress.... blah blah blah.

The school cannot on their own make the provision required to meet my child?s needs as despite ds having an assessed IQ of 121and being on school action plus the school have told me he has made no academic progress this year. (do you have evidence of this btw? Are the school likely to change their mind under LA pressure?)

This lack of adequate progress demonstrateds that leads the school are unable to give ds an adequate education that takes account of his special educational needs without additional resources and support.
