just got the renewal pack, (over 16 one)
all looks ok
but I cannot for the life of me think who to ask for supporting letters and who to do the statement in it.
she has a SALT/PHISIO/OT/TEACHER/TA
DOCTOR
????????

our GP, cardiologist, someone I know who works for contact a family or the social worker does ours, though he is just nearly 10.

I have just emailed her SW to ask him to do a letter, hopefully he will help me with the form.
so would the doctor be good(she isn't her gp, I am just not sure of her real title)

yeah, any of those you listed, though I always go for who knows him best.

I have just got my award HR care and HR mobility an indefinite award.I got the head of the autism base to fill in the statement from someone who knows him. They didn't approach his paed and got the ward within a fortnight of posting it off. I found the adult form easier for us because even DWP can't fool themselves that his abilities and behaviour is in any way age appropriate.

the form itself doesn't worry me, as it is kind of easy as she can't do any of it iynwim, so no if buts of maybe's.
night is a bit harder as she does sleep, but on a bad night if the melatonin doesn't work, or she is excitedlike xmas eve she won't sleep.
I hate the fact that they send it now. as it will take a couple of weeks to get supporting letters, then it will be the mad xmas post, then to send or not

I never send supporting letters probably because ds sees no health professionals. Paed last saw him aged 9,hasn't seen a SALT or psychologist since before that. I think he last went to GPs three or four years ago as he's as fit as a fiddle.
Would definitely write down the scenario of a night without melatonin though. Good luck oh and send it recorded or online perhaps.

ASDX2 and 2 shoes sorry to highjack thread but can I ask if you think it is worth my applying for High rate mobility when my DS (ASD diagnosis) is three next month. I have just this week been awarded High rate for care so don't want to jepordise that but feel my son meets requirement under meantal ipairment and danger to himself outside, no awareness etc

Yes I would give it a go too Mompa. Ds has had mobility since he was five (it wasn't available at three back then)It was a fight back then but at tribunal they decided he had an arrested development and it's gone through without a hitch since.
2shoes I think you'd have to send then in separately or fax them if you are able.
