Saintly can i ask a question as i know you know lots about this. What is the lactulose/mannitol test? My DS1 (dyspraxic, ?Aspergers) was tested for lactose intolerance and coeliac, but came back as fructose and sucrose intolerant - still has the diarrhoea issues as in the results section. DS2 i'm sitting on the fence as to behaviour, but came back lactose intolerant and so did DS3. They both still have symptoms as per the results section. If they have all come back clear for gluten intolerance, would the lactulose/mannitol test show if there was an issue with gut permeability? And therefore whether a GF/CF diet would improve their situation?

eek - that's a bit out of my knowledge area tbh. The lactulose/mannitol test is used to test for coeliac right?

All I can say really is that children who test negative for coeliac can sometimes still improve (parental report) on a gluten free diet.

There are some data sheets you can download to record behaviours which can help you assess whether there's a difference pre and post diet. And of course there's the autism research unit which will do the IAG screening.

Thanks. i'm at the clutching straws stage at the moment. Gastro paed and GP don't really have much idea of what direction they are going in. They keep changing meds (DS2), questioning behavioural issues (DS1) and just general consternation (DS3) but they all agree there is a real problem. The information on that paper seemed to tie in with my feeling that there is a genetic disposition that they are all affected by. Close relatives without diagnosis seem to have a higher % chance of also having 'gut permeability" and could be worth investigation.
