The 'How have you helped your child' bit.
What kind of things do they expect to see?

Perhaps you could add things like any therapy you do at home. Ds for example i have SALT exercises, sensory diet, OT exercises, visual exercises, feeding charts.

Any extra reading, spelling or extra help you give, or how you cope with behaviour at home.

I used SALT exercises to show that despite help from school, help from home, ds was still not meeting targets, therefore he needed specialist input.

I only filled out sections A, B, C, D and included help i give at home in these sections.

So mine went A early years, with lots of sub headings and what help ds has received.

B what is your child like now, exactly the same as section A, he hasnt made any progress and regressed in many areas despite all the help he has been given in school and at home.

Then C and D.

I didnt use the form at all, i put into a word document and just copied the headings etc.

The form is ridiculously small isnt it!

Also you dont have to answer every single question, just the ones you feel are relevant and back up what you are saying.

I paid particular attention to what help i thought ds needed and took it from there, adding things which back this up and evidence.

The help you give your child doesn't have to be formal therapies, it can also include the ways you modify routines and ways of communicating. I also said that I'd helped by listening to professionals' advice and taking him to appointments/assessments etc. You'd think that would be a given but you do need to spell things out sometimes!

I also used the section as a way of describing some of the strategies I used at home which helped DS, which would have been useful if the school implemented it. I know that his new school read my Parental Advice, because they've taken a lot of it on board and that's been helpful. Not so useful for the LA/tribunal but a good chance to pass on information which helps in the day to day work.
