Pee problem?! Or Poo

DS2 (ASD, Mod/severe learning diffs, hypotonic)
had a very nasty impacted bowel back in february half term which ended up with us in hospital for a week while he was unclogged. He's 13, verbal but doesn't appear to register pain until it's off the scale and can tell you his obsessions til you beg for mercy but can't tell you if his tummy hurts..:(

When it happened last time, docs were shocked at how totally impacted he was, as a) he didn't yelp when his tum was pressed, b) he hadn't noticed and we only realised something as wrong because he was peeing every 5 mins..literally..he got no sleep ..of course it was pressing on his bladder:(

It's happening again.. up all night and exhausted with the constant trips. He's on 2 sachets a day movicol and has gallons of water to drink, fruit etc etc . Had him checked today and yes he's loaded again but she doesn't think impacted yet.

ANY ideas how I can help him along? His lack of sensation means I don't think he really feels the need to poo 'properly' and his weak muscle tone means it has to drop out rather than push!

I HATE his bowel problems :(

Ds is prescribed lactulose and dulculax (sodium picosulphate), and he has loads of fruit, veg, liquids. Senna makes no difference and he won't take movicol.

It's horrible, but we also use microlax (enema)when things get bad.

I've been trying to get help with diet and behaviour therapy, because HE SHOULDN'T NEED ALL THIS MEDICATION JUST TO POO! But in the meantime, It stops things from getting any worse.

None of that probably helps at all. In short, I feel for you kidsncats.

Tallwivglasses, do you think it would hurt to add a bit of dulculax to the mix? (I have some in the house for me!)
I'm reluctant to go down the enema route only because a) he's 13 and b) last time the impaction was so high up that the hospital enemas didn't reach it! Shock but I'm thinking DS may need some extra stimulation to go with the movicol.

Lactulose just made his overflow poo burn his skin when he couldn't reach the toilet in time...

It's horrible isn't it? I so hate not being able to make it better for him.
