Mildy dyslexic/dyspraxic 13 year old - how do I motivate her?

My DD is currently at a selective independent school in London that is not catering to her needs. She has mild dyslexia/dyspraxia and although she can read, write and spell adequately she is not good at taking notes in class or at understanding what is being taught. Her maths ability is also very weak. She has gotten to the point where she is completely unmotivated to learn, has no confidence in her abilities and has fallen well behind at school. She's also going off the rails a bit at home and can be cheeky and very defiant of discipline at home. I want to move her before the situation becomes irreversible.
I have considered homeschooling her but that would only be a temporary solution and she is not keen on this option. She wants to stay in mainstream education, but needs a school that will teach her in a way where she can access the learning.
Can anyone recommend a school in London or the home counties (private or state) that would be good for my 13 year old. Or perhaps a good private tutor.
Any tips on what to do will be appreciated.
