AS DD w/other health problems struggling

My 11yo DD (AS, LD, heart probs, epilepsy)went up to secondary in September from an abysmal primary. The school are helping her tons with her work, her levels have gone from below L1 in maths in July at primary (she didn't acheive a level on her SATS) to lvl 2H in October, and higher now. Her english level has gone from level 2L to level 3M in Oct. The problem I'm having is that the school don't seem to be doing anything to help with her 'social development'. She doesn't understand the way the other pre-teen girls friendships are 'fluid', she either is friends with someone or isn't. She hasn't been eating lunch as no-one explained to her (or me!) how she was to get her free school meals (in my naievety, I assumed it was no different to primary as I was given no letters to the contrary, so I was giving her money to pay for lunch after the first week). She still can't tell the time or add up money, and she's getting detentions for being late in from lunch, or late to lessons when she can't find her way to the classrooms. The SenCo is only ever available for appointments at 2.30pm when I'm a lone parent and have my 6 & 7yo DS's to pick up from primary, so can NEVER see her (no-one else to pick boys up), seems to be getting worse, don't know what to do, don't want to kick up too much of a stink as they're working wonders with her educationally. Any suggestions?
