Is the school SENCO supposed to fill out the strengths and difficulties questionnaire?

Completely new to all this, we’re starting the process of having DD14 assessed, but because she’s 14 there isn’t time for the school to do it as she’s halfway through Y10 now. The school has given me a sheaf of paperwork to give to the GP to try and go through them instead and for some reason the school SENCO has filled out the SDQ, now, to me, it seems obvious it’s supposed to be filled out by parents/carers. The questions are worded ‘Your child’ and ‘Your family’ etc. The SENCO has no idea how it’s impacted our family life, and some of the answers she’s put I don’t feel are accurate, so I’m really confused as to why she’s done it!

Also when she signed it, she spelt her own name wrong Confused

The SDQ questionnaire can be completed by school, parents &/or DC themselves. Sometimes more than one will complete and answers compared.

At Y10, the school can refer. DD is unlikely to still be there when assessed, but they would be able to refer.

Typically, SDQ is filled out by a school, parents and the child. You can fill out a copy too. It's used to triangulate difficulties in different settings.

This isn't a universal approach but explains why the SENCo will have got it filled out.
