Year 5 SEN plan

I am desperate for advice.
My daughter has a SEN plan, no known reasons for the learning difficulty- I have had her assessed for dyslexia but they said she doesn’t have that. She does have a processing issue I may have posted before about, I’ve basically had a report about it and it’s never been mentioned by school again.

I’ve just gone through some of the paperwork I have, she’s had the same targets since beginning of year 3 at least (she’s yr 5 now) and she still hasn’t completed any of them. I’ll get her new plan within the next week but from doing homework with her I know she hasn’t secured them still.

I don’t know what to ask the school to do, break down the targets maybe because they’re quite long, the closer we’re getting to secondary school I’m really getting quite anxious for her. Her targets are year 2 based so she’s significantly behind. What are the next steps when there’s just no progress?

The school are still showing no concern about her ability, they play it down at every meeting and say it’s not really a problem.

Was the report an EP assessment? Is it an EHCP she has if you are in England? Or just a school based plan/ IEP?
You are right that they shouldn’t just keep giving her the same targets without breaking them down or using strategies and support to work towards them.
Targets should be challenging and aspirational but also there should be SMART targets. They don’t sound realistic or achievable which isn’t good for her progress or self esteem.
You shouldn’t have to tell school what to do. They should be differentiating and the SENCO should be supporting staff.
You could ask whether there are any outside agencies involved e.g. specialist teacher. Has she been assessed by SALT recently?
If she doesn’t have an EHCP you could request an EHCNA.

This is just a SEN plan so similar to an IEP. She does get differentiating work, but I just don’t understand why there’s been no progress in 2 years, and why this hasn’t been picked up by anyone at school, the same SEN plan literally just gets renewed word for word every term and sent back to me.
thank you for the SMART targets comment, that’s exactly what I was thinking she needs so will pass that on to the teacher.
She did see SALT a year ago who picked up on the processing difficulties, I will ask if a follow up is possible because other strategies obviously aren’t working. The school were very put off when I mentioned an EHCP to them last year and were adamant she doesn’t need one, and other than these SEN plans (which are all word for word the same) I don’t have other evidence to send off for it myself.

I’d get some advice from the local parent SEN support service. Look on the LA Local offer.
Lots of advice and support online for requesting EHCNA as a parent.
If you don’t get anywhere with the SENCO or teacher try the Head or contact the SEN Governor. They can’t just keep saying a 3 year delay is nothing to worry about.

Request a meeting with the SENCO. The school needs to be providing support. What support are they providing and what have they tried?

Request an EHCNA yourself. On their website, IPSEA has a model letter you can use. Ignore the school trying to put you off. Sadly, some schools incorrectly tell parents their DC don’t need or won’t get an EHCP. You might have to appeal, but it is possible to get an EHCP.

DD needs an assessment by an ed psych so that her needs and the provision she requires are understood. She needs an updated SALT assessment. An OT assessment would also help.

Support on the SEN plan is very basic, allow time to process questions before having to answer, sentence starters etc. The past 2 weeks they’ve been putting her in a smaller group for most lessons, I think there’s 4 kids who get differentiating work who move to this room with her. I will ask for all the assessments, and let the school know I’ll be applying for an EHCP, they do have a way of making me feel dramatic about lack of progress when we have meetings so I’m going to try and take someone with me as support!
