I don't know how to get the ball rolling

I had a meeting with DD7's teacher due to her impuslive behaviour and lack of focus in school. It's been an issue at home too. Her teacher agreed that I should assess for ADHD. I've done research since the meeting and rang the GP who has stated she'd need an in-person appointment. But if I go down right to choose pathway, should I be asking school to complete the SNAP IV form prior to the initial appointment? If I use the generic NHS one will they ask me to complete a specific one for the organisation I use for assessment? I don't want to seem like I'm wasting their time.
Am I getting too far ahead of myself and should I just have the initial appointment for referral or will I need to mention right to choose there and then?
Sorry if this sounds ridiculous. I know no one in my life who has a child with SEND to ask questions and even though we've been suspecting it for a long while, I still feel kind of shocked and really alone in this. I'm so scared people (including the doctor) are going to just roll their eyes at me and tell me it's my parenting that's the problem, that I feel frozen in what to do

If you want to go down the RTC route, it would be a good idea to know which provider you would like DD to be referred to beforehand. What each provider wants as part of the referral will depend on the provider. Some don’t need any questionnaires until after the referral has been made.
