Do I need a doctor's note?

Hi, I'm after some advice. My DS's school (academy) say they will not take into account his needs, or make adaptions or concessions without a doctor's note. Is this right? I thought the Children's and Families Act 2014 said a diagnosis was unnecessary and it was the impact on the child that mattered? It also says in the SEND Code of Practice at Section 6.15 – “A pupil has SEN where their learning difficulty or disability calls for special educational provision, namely provision different from or additional to that normally available to pupils of the same age.” Thereby implying a formal diagnosis is not needed. Have I misread this? Should the school make 'best endeavours' to meet my child's needs regardless of whether he has medical proof of them?
Thanks for reading.

IMO totally unlawful.

Make sure you have it in writing from them - and if you don't, send an email to the person that said it and get confirmation that this is what they believe. Then copy that response in your complaint to the head/governors and quote the law.
Then ask them what support they are going to put in place.
By not doing it, they are facing an allegation of disability discrimination.
Good Luck!
