SW has made some mistakes in her assessment, I sent her a very polite email asking her to do the corrections and outlined the corrections in order and in a very organised way. It is about dates, data and some facts that she got wrong / misunderstood, including things that happened in different schools - I don't think it is fair for a situation to be linked to a particular school when it happened in a different school for example - but there are other little details that change the whole picture and I would like her report to be as accurate as possible because:
a) my child will have access to the report to keep forever and when her memory fail I don't want her to think that the wrong facts in the report happened - it is confusing
b) we will apply to PIP and I will send this report (with other reports from other professionals), the current report doesn't reflect the extent of the situation and my child's struggles but it is important that it is accurate so she can receive all the help she can, including financially
c) other professionals will probably have access to the report and they also need to have a accurate understand of the situation
SW replied saying that she can't correct it as it has been signed off already but she can include my amends on the top of the assessment.
I hope she does but is this normal practice?
When I had other reports from other professionals, they usually send the draft 1st and are happy to take corrections in consideration.
If corrections / amendments are not done, where do we stand?
