5 week old screaming

My 5 week old has silent reflux for which we are giving him carobel, have tried gaviscon and omeprazole which made him worse but carobel seems to help at least slightly.
He also has terrible gas for which he has gripe water and infacol. He farts all day and night and will keep needing to bring up burps every hour.
Anyway up until now he has settled himself to sleep but for rhe past 3 days he's unable to sleep and screams and cries until he's held and rocked.
I didn't want to go down this road as my eldest was rocked to slp until age 3, my second I could just put down and ignore and she'd sleep. I've tried the same with this one but he literally goes insane and with 2 other dc who need to sleep I cant leave him to cry nor would I as he's still tiny, he's in a next to me crib and a carrycot in the day.
I'm pretty sure it's overtiredness but I've mentioned the gas and reflux in case it sounds familiar to anyone.
What can I do? I don't particularly mind holding him until he sleeps but I didn't want to have to rock him but unless I do he gives out the most pained sounding cries

Is he swaddled? My LO can't sleep without being swaddled and it's a good sleep cue x

Sounds very familiar to me and I suspected ds had CMPA (cows milk protein allergy) which was not ideal during lockdown 1 😬
Doctor prescribed nutramigen milk which helped almost instantly with ds. I needed to nag doctors about having this milk but so glad I did. It changed my little boys life as he was miserable for about 3 months when we didn’t know what was wrong.
We were also prescribed gaviscon which didn’t help much really.
Gripe water helped a lot during the night feeds and holding ds upright as long as possible after a feed as he also had silent reflux/acid (a symptom of CMPA)
We ended up buying a baby beanbag type chair that held him upright as he was getting heavy

Thank you for the replies I'm going insane from lack of sleep.
I do hold him up 30 mins after each feed as that's what ee did with eldest who had full blown reflux.
I do already give gripe water and he loves iy so that's a plus. The groaning and grunting during the night is insane too so even when I've got him to sleep I still cant sleep myself.
My dh takes over half the night but we both feel he's in pain.
I'm breastfeeding and I drink almond milk I always have but I do have chocolate and cheese so could that still cause an allergic reaction? I don't have much cheese maybe once a week if that.
I have fennel tea as thats supposed to help with wind too.

Is it worth going back to the drs? They seem reluctant to do anything

My breastfed baby was screaming for me as I was passing over nut protein in my milk, would.your almond milk be crossing over? Mine was a very large bar of fruit and nut each day, when I cut that out he stopped screaming and if I accidently ate something else nutty he would have a night if screaming again.

@stairgates I hadn't thought of that, I'll have to cut out almond milk too to see if it helps, I can easily switch to oat milk as we already have it at home or I could forego milk altogether as I only have it at breakfast.
I've not been having anything else nutty.
Hes so miserable the poor boy, he looked at me and smiled the first 3 days and since then he's just always un pain and fussing. I want my smiley boy back

If its similar, my little one would only poo once a day and it would be orange instead of the regular yellowy b/f baby poos. When I cut out the nuts it would take about 24hrs to make a difference to him so give it a few days before ruling it in or out, good luck it is heartbreaking

Oh my goodness,
Yes I forgot about the grunting.

I would go back to the drs and push for a milk that they prescribe.

My ds symptoms were:

Grunting when asleep (like a farmyard!)
Poor sleep
Flaky skin
Bloating
Bad nappies
Slept/settled better when upright.
Miserable!
Over feeding (which I later found out was because drinking milk helped to relieve the acid, but of course made things worse )

My doctor told me I would ‘know’ if my little boy had an allergy. I thought bloody hell, I’m miserable and so is he. I KNOW something isn’t right.
I had to literally keep pestering for weeks until I reached breaking point. I actually rang my Hewlth visitor crying and she got the ball rolling with things.

It’s awful and hope things get better

@Yolo773 i will get back to the drs then as my ds has a lot of the same symptoms.
He has very flaky skin too which I just thought was normal and nappy rash even though I change him asap. I cant believe I didn't put those things together Sad

I thought a cmpa was very rare when breastfeeding too so I've just thought it's bad reflux.
Everytime I go to the dr they make it seem like I'm being dramatic and it's all normal and he'll grow out of it but this is my 3rd dc!
I remember having to tell my Dr my eldest had reflux 10 years ago, up until then she'd kept sending me home saying he was fine but I knew he wasn't.id had to research things myself. She then agreed with me and put him on ranitidine and he was a different baby.
It's sad 10 years on things haven't changed.

I think CMPA is a lot more common,
Or perhaps misdiagnosed as reflux/silent reflux.
I’m no expert but as my ds was a lockdown baby I didn’t get much support to begin with (covid, lockdown and very difficult to see a gp or get anyone round to look at ds)
So I researched and researched and researched online and discovered lots of info on there.
There is a support group on Instagram (CMPA parents together) which helped me a lot.
Trust your instinct. Go to docs and tell them how you feel.
Ds was my first so I felt like I was being stupid to even suggest CMPA, and I was also made to feel like I wouldn’t know because I had no experience with a baby before. But as I say, I knew he wasn’t right.

Maybe be wary to cut out things completely in your diet before speaking to a health professional

Your body needs certain nutrients especially if you're breastfeeding

Have you tried your health visitor / GP?

I have to pester my gp just to even see us, they've palmed me off a couple of times already before I insisted there was something wrong.
Hes got really flaky eyebrows this morning and had diarrhea so there's something definitely wrong

Just an update in case anyone else is going through something similar.
I went back to the dr and they agreed to give him liquid omeprazole instead of the tablets that have to be dissolved as he kept spitting that out.
I've also started him on probiotics he's had thses for a week now.
Since then he no longer grunts half as much as he used to which means when he's asleep I can actually get some sleep myself!
His acid reflux seems better although not gone entirely and we still use carobel and hold him up for ages after a feed. He is still feeding every 2 hours but has gone 3 hours a couple of times so I'm hoping he will start to space them out more especially at night.
I have also cut out cows milk so maybe it'd a combination of all of these but I'm not going to stop any of them as I don't want to go back to the screaming!
