8 month old, have no idea what I'm doing

Please help. My DD2 is 8 months old and a terrible sleeper and I don't even know where to start to make it better.
She's breastfed, she refuses all bottles and will only take a dummy to play with when she's in a good mood.
She is very difficult to put down for a nap and even if I settle her she is liable to wake after 10-15 minutes anyway. I prefer to settle her in her cot but this can be difficult when I'm on my own with a 2 year old as well, so often if she's asleep I will put her on her rocker in the living room. If she falls asleep downstairs I cannot take her upstairs because putting this baby down is a delicate procedure.
She does occasionally surprise me with an hour nap but this is unusual. She rarely falls asleep in her pushchair. She does fall asleep in the car seat but 95% of the time she falls asleep on the boob. Nobody else can settle her to sleep.
At night, she prefers to co-sleep, which I hate because I don't get any quality of sleep when we do this. I often give in through exhaustion, though. She usually wakes every 2 hours but if she is unsettled (teething, illness, growth spurt) this will reduce to every hour until I bring her into my bed.
I know I've made all sorts of problems for myself here and I don't know where to start with fixing it. Should I put her on a schedule? Night wean her? (I actually hate breastfeeding at this point).

Any movement away from breastfeeding to sleep and co sleeping is going to involve a huge amount of distress - probably for both of you. There will be lots of screaming and crying. Can you cope with that? Is it something you are willing to tolerate and do you have support (partner or your mum?) through it?

Alternatively, the easiest and gentlest answer may be to find ways to make co sleeping work and maximise everyone's sleep, while slowly edging yiwartowards independant sleep.

A sidecar cot might be a good start point. Not co sleeping in your bed Then, but still within arms reach. Just remove one side off the cot and wedge it up to your bed for this. And if daytime naps in the rocker are easiest, just do that and don't stress it.
