Help me get my refluxy 6mo to sleep :(

He's finally finally respondng to some medication (omeprazole) but he's still not 100% pain free, so I've been overly cautious when it comes to leaving him to self settle as I've never been confident that he's not in pain. He will only be fed to sleep, or on occasion rocked vigorously. (He'll sleep in the sling/buggy during the day.) Consequently he is rarely hungry during the day and we are struggling with solids as well as getting milk feeds in during the day.

As a result, he is the worlds worst sleeper. An average night looks like this:
7-730pm asleep
10pm awake, feed to sleep
12am, 2am, 3.30am, 5am, 6am all fed back to sleep
7 up for the day

I can't co sleep as he needs to be upright for his feeds and for a period afterwards, and his bed is at an incline. He also sleeps in a sleepyhead pod, will only sleep for 20 mins at a time without it.

During the day he will feed to sleep around 9 am for 30-45 mins, then sleep on the go around midday for an hour or maybe 2 if I feed him back to sleep after an hour, then another 45 mins around 4pm. He is grumpy all day long with tiredness. We have just about eliminated pain as a source of his daytime grumps but now he is just grumpy with tiredness instead.

This pattern changes a lot though depending on older siblings school runs etc.

I am in desperate need of more than 60 mins sleep at a time. I am on my knees.

I am v much against CC and feel that he is too young to night wean, I am happy for him to feed 2 or even 3 times, but 6 is ridiculous. After each feed it is touch an go as to whether he'll re settle, sometimes he'll be up for an hour or so with lots of bouncing/rocking etc.

Where do I start? I am so devoid of energy this seems like such a huge challenge.

Does anyone have any ideas? Last night he woke every 20 mins between 2-6am...
