Books to explain the notion of death/ disability & difference to a child of 5?

My friend's grandmother has just died and her son is asking lots of questions about death and dying.

Can anyone reccommend any books which might help her to explain this to him?

I'm also looking for a bit of help as our dd has a bladder problem and is starting to feel really different to other people. Her younger sister is disabled and I feel it would be good to explore the idea that 'everyone is different'. If anyone can suggest any books which might help with either of these issues, I'd be really grateful.

can't help with death but we just watched 'how to tame a dragon' which is pretty positive re disability. dragon is disabled, needs some special equipment to fly, and without wishing to spoil it for you, the young lad loses his leg (being brave) and is given a falsie. all very ho-hum that's the way the cookie crumbles and sparked some interesting chat with dd1.
