Condom reservoir ends - why so small?

Having been pregnant more than once whilst using condoms and relying on them once again, I'm curious as to how they fail when they have not split or broken (as was the case with my 2 pregnancies as far as I know).
Bearing in mind that the average sperm ejaculation is 8 to 10ml, why is that little teat on the end of a condom so small - is it a gimmick?
DH and I had sex the other day and I immediately inspected the contraceptive device after withdrawl and whilst he was still hard - the reservoir was fully inflated and I could see sperm welling up the sides. No wonder I have 2 kids!
Surely, if it is intended to hold the full delivery, it should be bigger?
Having said that, DH says he can feel "the flappy bit" and wished they made them without the teats (he says it feels tight at the end) - he is on his own if they start making condoms without any expansion chamber.

You're supposed to squeeze the end a bit when you put them on, and not roll them on right from the end, so there is some space...

I’m surprised you’re relying on condoms when you’ve been pregnant twice before using them. Sounds like you need another form of contraception.

It's not on properly the semen shouldn't be getting pushed up along the sides of the condom

I would try different condom brands/types. I prefer ones that have no teat at the end, but instead are basically wider at the end. They have more room for the ejaculate.

Brands tend to have sizing and shape information on their websites.

edit - I'm sorry the responses seem to mainly be telling you you're doing things wrong. Condoms, like penises, come in all shapes and sizes and sex education should tell people that it's essential to find the right fit.
