Mainstream school for mild SEN teenager

Can anyone recommend a school (private or state) that would be good for my 13 year old. She is currently at a selective independent school in London that is not catering to her needs. She has mild dyslexia/dyspraxia and although she can read, write and spell adequately she is not good at taking notes in class or at understanding what is being taught. Her maths ability is also very weak. She has gotten to the point where she is completely unmotivated to learn, has no confidence in her abilities and has fallen well behind at school. I want to move her before the situation becomes irreversible.
I have considered homeschooling her but that would only be a temporary solution and she is not keen on this option. She wants to stay in mainstream education, but needs a school that will teach her in a way where she can access the learning.
Can anyone recommend a good secondary school in London that caters well for children like my daughter? We will also consider schools in the home counties as well. Thanks for any suggestions.

The Good Schools Guide to for SEN is a very good book recommending various schools. They include both mainstream and special schools but there are quite a few mainstream, academically selective secondary schools there.

The Gabbitas guide is a bit more up to date, it has a section for mainstream independent schools and shows which kinds of SEN it has previous experience of.

Hampstead School is very good. Has a large and well resourced SEN department.
