Declaring SEN on independent school form

Completely new to this! But I need help desperately.
Currently filling out a form to start my daughter in an independent school for year 7.
I have self referred my daughter to Camhs (18months ago) due to suspected neuro diversity and the stage we are at now is that she is awaiting an assessment to see if she has asd.
On the whole is does not affect her schooling. She is bright, has friends and is well behaved. Her struggles are mostly out of school presenting anxious and a lot of sensory issues (which is why we chose independent school with small classes etc) She has ticks sometimes but this has not been picked up in school.
My question is What do I put on the form? I don’t want to play it down and know I need to be open and honest but currently it doesn’t affect her school day however my worry is as she gets older socially it may be an issue (she has no filter) & although she goes into school fine she would much rather be at home with me so there’s that resistance to going into school.
The question on the form is- does your child have any health issues diagnosed?
Im Just not sure how to word it so that they know there is potential neurological diversity however nothing that would need any adaptations currently.

You need to ring the school and speak to the head / senco / admission person. This will be the best option for your child. Equally if you named the school then people might able to help / tell you how they found the school with a sen child

Has she got an offer for this sept?

@seaisamazing Thank you for your message. She has an offer for this September yes. Thank you I will have a chat with them and explain- it’s so hard to even put into words sometimes; in school there’s no issues and she “gets on” but it’s definitely something that needs highlighting. Thank you so much.

I would also say to check the form for the SEN section and do write honestly everything in it - I know a parent who an indie got very cross with because they thought she had "lied" on the application form...Just be wary not to rub the SEN dept up the wrong way by being in any way mis-leading.
