Independent school application for boy with suspected Asperger

We are currently in the stage of registering DS for some super selective independent schools for 11+ entry. Some of the registration form ask something like this "Does your son, or do you believe your son has, any medical condition, health problem, allergy, learning difficulty, disability, special educational need, behavioural, emotional and/or social difficulty?" with a list of ADHD, dylexia, Autism, Asperger and etc. I do believe DS has mild asperger (my dad has it at a more server end of the spectrum) but we never got him formally diagnosed. He is bright and generally ok in school except recent anxieties developed when back to school after lockdown. Should I tick the box in the registration form? Will this affect his chance of being accepted to super selective schools?

if he has not been diagnosed, I would not disclose it. I would check though what then schools general pastoral care and SEN support setup is. Some have a very good set up where they screen all children for suspected difficulties in Y7. If you are applying to superselectives, it is very likely that there are plenty of other undiagnosed Aspies at those schools. I am fairly sure the boy who showed us around St Paul's at the open morning was on the spectrum, not sure if diagnosed or not.
