Ex partner still won't move out.

I'm at my wits end, with him now.
I called the police in November, but all they did was caution him and bring him back here. Even though it's my name on the tenancy and I have learning disabilities and mental health problems.
I can't afford to change the locks and I can't take much more.
Please help.

Who can advocate for you ? Do you have any sort of Social Worker or Key Worker ? Are you supported with your mental health by any groups ?
The Police need to help you to remove him. Speak to Women’s Aid, they should be able to help.

You can buy a key barrel from B&Q for about £10-20. You tube how to ‘change master cylinder’ - with one screwdriver - it’s one screw and really simple / easy.

He never goes out the house for me to change the locks or speak to women's aid, as there not supposed to be in the house when you ring them.

Big burly male relative to tell him to move out and help you change the lock. If your ex isn't paying rent or anything he has no legal right to be there. I wish you all the best

Can't you say no to the police when they try to return him? No its my house not OUR house?

I tried saying to that to the police, but they said this is where he lives so they were bringing him back here.

I've been in this situation and I can tell you it isn't easy. Mine was verbally abusive and threw all my belongings outside. Police said unless he hits you we have no choice but to bring him back here as it's half his house. Fuming I was
