If its an official document to be read by a judge, I would be upset about the language used. I would be working towards getting this changed to read exactly what you said. If the original recording was destroyed how have they managed to make up this statement?

It's not my field, but I would imagine you need to identify all the things that are incorrect and dispute them in writing, making it clear that you were under the impression recordings would be deleted after the reports had been agreed and filed.
